Dear Parent / Carer,
In line with government guidance we will have a mass testing centre at the school to test all staff and students for Covid-19 on their return after the Christmas break. We therefore would like to test all students attending the school when we return in January.
The Covid-19 lateral flow testing that we are offering is designed to identify any individual (student or staff) who is asymptomatic (not displaying any Covid-19 symptoms) so that they can be isolated from the school setting following a positive result to reduce the possibility of other students and staff in school becoming infected.
In order to offer your child a lateral flow test we require your consent as their parent / carer.
If you have already provided consent previously to lateral flow testing in school you don’t need to do it again, but if you now wish to change your consent then please complete the form.

If you have more than one child who currently attends The Radclyffe School you will have to complete the consent form for each child which will mean that you will have to log out of the first child’s school account and then log in with the next child. A link and instructions on how to do this will show on the page after you submit the form.

It is important to be aware that students and staff who are showing ANY of the main symptoms of Covid-19 must still isolate and book a Covid PCR test directly with NHS Test and Trace and NOT send their child into school to get tested.
